Children's Case Management

 

MOBILE ASSESSMENT

is a collaborative effort with the Columbia County Department of Social Services, the Columbia County Mental Health Center, and the Columbia County Youth Bureau. The program assists families of elementary and middle-school children whose personal or school success is in jeopardy.  By using family strength-based conferencing, involving all service providers assisting the family as well as the family’s own supports, the program creates a plan to meet child specific needs.

COMMUNITY-BASED PREVENTION [Columbia County]

provides services for youth at high risk of out-of-home placement who will benefit from service coordination.  A family worker provides support, linkage to service, advocacy, direct care (transportation, tutoring, role-modeling) and flex funding for necessities to keep the child and family safe and healthy.  The  goals include improved behavior and school attendance, reduced out-of-home placements, family members linked to appropriate services, and improved communication between school and family members.  Services are provided until the family members attain goals and are linked to appropriate service providers to maintain the health and safety needs of all family members.

COMMUNITY-BASED PREVENTION [Greene County]

helps children with serious emotional disturbances by providing support to families and linkages to appropriate services in the community.  The goal is to prevent out-of-home placements by providing case management support to families.

SCHOOL-BASED PREVENTION (SBPP)

(Ichabod Crane and Taconic Hills)

provides two home-to-school coordinators in each district offering services for identified students (K-middle school) who exhibit behaviors in the school that may have a basis in the home or where the school has experienced difficulty engaging the parents / primary caregivers.  The SBPP array of services to families includes support, linkage to services, advocacy, mentoring, direct care (transportation, role modeling, tutoring) and flex funding for necessities to keep the child and family safe and healthy.  Emphasis is placed on establishing partnerships between parents and service providers working together to secure services based on each family’s strengths and needs.

FAMILY ADVOCACY

staff are often parents of children with an emotional, social or behavioral disorder who facilitate support for children and families in the community, on local and state committees, and at conferences relating to children's mental health services.

COMMON SENSE PARENTING™

 is an evidence-based practice providing six 2-hour skill sessions for all levels of learners and for parents of all age children.  Skills include effective praise; staying calm and teaching self-control to children; building a close parent-child relationship; and how to give clear messages with expectations and consequences.

PARENT SUPPORT

affords parents, children and adolescents (17 and under) with emotional, social and/or behavioral disorders an opportunity to support each other on a regular basis in a safe, confidential environment.  Child-care and transportation are provided whenever possible.

ADVANTAGE AFTER-SCHOOL   

provides fun, structured activities for children and youth within the Hudson City School District in grades K-8 at the Hudson Middle School and the Hudson Youth Center on school days during the academic school year.  This free program, open to all students, provides a variety of activities including homework assistance, fitness programs and arts and crafts.

TIER I FACILITATION [Columbia County]

is a family advocate facilitated, strength-based conference to develop an action plan for families and service providers where there is multi-system involvement.  Through use of the family’s existing assets, the family members, family advocate and current service providers focus on setting attainable goals, assigning tasks and clarifying roles.

RESPITE (Planned, Hourly, Crisis)

workers implement activities for children and adolescents (17 and under) with an emotional, social and/or behavioral disorder.  Children of adults with a psychiatric disability are also eligible for respite services.  Respite arrangements are designed by the families and implemented as a positive self-esteem building and role-modeling opportunity for the children.

FAMILY DEVELOPMENT CREDENTIAL (FDC) TRAINING

is offered for front-line family workers to promote strength-based, empowerment skills in all facets of delivery of service to families.  Workers learn how to assist individual family members in setting and taking ownership of their goals.  Workers also learn the benefits of collaboration and cooperation.
