Clinical and Rehabilitation
CONTINUING DAY TREATMENT (CDT)
is an OMH-certified program providing intensive outpatient treatment for adults with psychiatric disorders including those with addictions and trauma backgrounds. Specific services include medication evaluation, a variety of individual and group therapies, very limited case management, crisis intervention, psycho-education and skills training including expressive arts and evidence-based holistic practices. Individuals admitted can use services up to 5 hours per day 5 days per week.
An on-call service is available when the program is closed.

INTENSIVE PSYCHIATRIC REHABILITATION TREATMENT (IPRT)
is an OMH-certified program assisting persons diagnosed with mental illness to achieve success and satisfaction in chosen life roles. Students develop interests and internal resources, explore the community and its resources, and focus on social relationships in work, in school and at places of residence. Currently, individuals admitted can use services up to 5 hours per day 2 days per week and can attend program series more than once, as specific program content may change.

SUPPORTED EMPLOYMENT
is a vocational rehabilitation program assisting persons diagnosed with mental illness and those referred by department of social services. Staff consult with clinical providers to provide coordinated and well integrated service plans. These individualized plans include vocational and employability assessments, job readiness classes, individual job search, job placement, job coaching, field advocacy with employers, assistance with financial and health benefits, and networking with community support agencies. Upon vocational stabilization, staff sustain supportive follow-along services. This program is an OMH-designated evidence-based Supported Employment practice site, and also serves under a VESID contract. The program has been granted funds to serve individuals under a NY State L.I.V.E.S. Grant.

BLENDED CASE MANAGEMENT [Greene County]
is an OMH-certified program targeted to serve high-risk, high-need adults with serious and persistent mental illness. Services are delivered by community-based collaborative intervention to increase community tenure and avoid unnecessary hospitalization.
The program develops individualized plans to problem solve and overcome obstacles faced by consumers with a focus on rehabilitation and service linkage. This program is linked to Single Point of Access (SPOA) and is available 24 / 7.

SUPPORTED EDUCATION
is a collaborative program with Columbia-Greene Community College to provide support, advocacy and mental health education to students with psychiatric needs at the community college campus.

NEW LEAF SOCIAL CLUB
provides a supportive and positive environment for peers in the Columbia County system of care. Staff interact with members of the Club in a facilitative manner, providing both structured and unstructured individual and group activities.

COMPEER [a component of New Leaf Social Club]
matches Club members with staff to serve referred adults who are diagnosed with a mental illness. Meetings are held regularly to engage in enriching individual and group activities.
