Referrals & Information

Many of our programs require a referral or other registration process to get involved. Below are some of the referral and registration forms for various programs.

MHACG Programs

We go beyond mental health to support with your overall wellbeing. We have services to support survivors of assault, crime and abuse, substance abuse and recovery, supportive residences, respite, mentoring, after school programs, peer support, and strengthening families.

Referrals are not required for REACH Center services; call 518.943.4482 for our 24-hour helpline assistance. If you would like to refer someone for support you can complete and submit the form below.

Residential Services - Supportive Housing

Supportive housing application process is managed through Single Point of Access (SPOA) for both counties. To speak with someone at our facilities for more information or support, please call 518.828.1068.

Important: The SPOA application form must be completed for the correct county and sent to that respective agency as outlined in their information. Please do not send to our offices.
